  • Who is Sterling Wellness Solutions?
    Sterling Wellness is the new wellness partner for the Nuvance Vitive Health Wellness Program providing the portal for wellness activities and points, health screenings, coaching, tools and resources.

  • What is Health Park?
    Health Park is the name of the new wellness portal provided by Sterling Wellness. The portal will allow you to track your incentive progress and access tools and resources customized to help you reach your health goals. You can sync a fitness device, access interactive health education, participate in health challenges, connect with a coach, find social support, and more. Many of the activities in the portal can also help you earn points toward your wellness incentive. Please note that the Chrome browser is suggested for the best user experience.

  • How do I log in to get started?
    You can access the new wellness portal at MyVitiveHealth.org.

  • Who is eligible for the wellness program?
    All Nuvance Health employees are eligible to participate in the wellness program. Employees covered on the Nuvance medical plan who complete the incentive components by October 22, 2021 will be eligible for savings on 2022 health insurance premiums. Employees who join the plan after September 1, 2021 are not required to complete the program to receive premium savings. Legacy HQ staff governed by a collective bargaining agreement are not eligible to participate in the 2021 Vitive Health program.

  • How can I qualify for savings on my health insurance premiums by participating in the Vitive Health Program?
    You can save $400 on annual health insurance premiums if you earn 400 points by completing a Health Assessment and Health Screening. You can save an additional $200, for a total of $600 in annual savings, if you earn 200 points from wellness activities.

  • What is the deadline to be eligible for wellness incentive savings?
    Health screenings must be completed by October 8, 2021. The deadline to complete the Health Assessment and other point-earning activities is October 22, 2021.

  • What does the biometric screening panel include?
    The onsite health screening events and the offsite voucher will consist of a 38-panel blood screening, including a cholesterol panel, glucose, Hemoglobin A1C, complete blood count, and complete metabolic panel. Your blood pressure, weight, and waist circumference will also be collected.

  • If I miss the onsite health evaluations, can I still participate in the program and complete a health screening?
    Yes, you may request a voucher on the Assessment page at MyVitiveHealth.org in order to complete your screening at a nearby participating LabCorp. Remember, health screenings must be completed by October 8, 2021.

  • How will I receive my lab results?
    Test results are typically accessible in the new Health Park wellness portal 2-4 days after you complete the screening. Click on Know Your Numbers to view your results and corresponding education to help you understand your current health.

  • If I’ve already seen my doctor and had blood drawn, can I use that to earn my 200 points for my Health Screening?
    Yes, health screenings completed with your provider between November 1, 2020 and October 8, 2021 are admissible for credit. On the Resources page in your new Health Park wellness portal, you can download the form to be completed by your healthcare provider. To submit the form and a copy of your results, select the Physician Submitted Lab option on the Assessment page.

  • Where do I find my incentive progress and points status?
    Log on to your wellness portal at MyVitiveHealth.org. On the Home page, you will see a status bar showing your progress toward earning the incentive. Click on the “TRACK YOUR INCENTIVE PROGRESS” button to view a list of points earned and remaining opportunities.

  • Will the points I earned under the previous vendor show in the new portal?
    As we transition to the new Health Park portal, we are working toward migrating the points that members previously earned under the previous provider’s portal. If you completed your Health Evaluation with the previous vendor, you will see that the Health Assessment and Health Screening components will be marked as complete in Health Park. The Vitive Health team is also working on a process to award points for members that have completed activities that have not yet been added to the portal.

  • What if I have a condition that prevents me from completing a Health Evaluation?
    If you have a health condition that prevents you from completing the annual health screening, use the medical waiver form in your Health Park wellness portal to provide documentation from your healthcare provider by October 22, 2021.

  • Who has access to my information? Does Sterling Wellness Solutions share my results with anyone?
    Your personal health information is treated with the utmost regard and is not shared without your express permission according to HIPAA and EEOC guidelines. No personal health information will ever be shared with your employer. De-identified population level data is shared in aggregate format to help guide programming that meets the needs of the Nuvance Health population. Your Vitive Health team will only have access to participation level information.
