Northwell Health Clinical Call Center: 516-918-6005

Northwell’s Clinical Call Center is available 24/7/365 by nurses certified in emergency medical dispatch who will triage your call and provide clinical support. When you have questions about the level of care you need, the Clinical Call Center nurses have the answer. Some examples why you might need to call the Clinical Call Center include:

  • You have an urgent clinical question or worry - like about the dosage of your medication
  • You have a new or worsening symptom
  • You don’t feel well and are unsure if you should go to the doctor or urgent care or the emergency department

At Northwell Health, we want our employees to receive the best care possible – and that includes navigation through our health system, personal health coaching, and 24/7 access to a registered nurse for clinical advice. These services are offered by Northwell Health Solutions at no cost for team members (and their dependents) who are covered under the value and buy-up medical plan through UnitedHealthcare (UHC) and High Deductible Health Plan (HDHP) through MagnaCare. You have our support when you or your family has a healthcare need. We want to help you focus on your health and recovery. The better we care for you, the better you care for others.


Healthy Living Program: 516-918-6009

If you're living with pre-diabetes, diabetes (type 1 or type 2), high blood pressure, high cholesterol, or are overweight, Health Solutions offers nutritional and lifestyle counseling through individual appointments with a dietician, as well as group classes. These sessions, which are covered 100% with no copay by the UHC Northwell value and buy-up plans and MagnaCare HDHP plan, will teach you healthy eating habits, methods of coping with health challenges, and promote a better understanding of medications and your progress.

Care Navigation and Coordination Programs: 516-918-6009

If you are hospitalized or looking for extra support managing an illness, Health Solutions’ teams of nurse practitioners, registered nurses, and resource coordinators can help you:

  • Understand your medications and condition
  • Navigate to care you need
  • Coordinate your appointments, and work with you to identify questions you want to ask your doctors
  • Create an individual care plan designed around your goals
  • Connect you with programs and support groups that may be beneficial to you, and find resources to help you manage your health

New! Behavioral Health Navigation Program: 833-673-0020

The Behavioral Health Navigation Program is a team of licensed behavioral health counselors who provide a confidential concierge service at no cost* for team members and their dependents in the value, buy-up and HDHP plans looking for mental health care. Identify mental health providers to fit your needs and fast-track an appointment with select Tier 1 Northwell providers without copays at 833-673-0020.

Diabetes Care Management

Northwell Health’s Healthy Living Program is a team of health coaches, registered dietitians, and certified diabetes educators who can empower employees with diabetes to take better control of their health. The Healthy Living Program is accredited by the American Association of Diabetes Educators and certified by the CDC to offer the National Diabetes Prevention Program. The Program offers individual appointments with registered dieticians for an assessment and coaching around an individual’s goals and needs. The Program also leads classes on nutrition and lifestyle counseling to support employees beyond individual appointments.

Team members who participate in Value, Buy-up and HDHP can self-identify as a patient with diabetes and express interest, or Health Solutions care management program will reach out and ask team members to join the program. In order for this to count towards the The Well-being Credit Program, participants must also complete a primary care provider wellness visit (this will give you another wellness credit) or endocrinology visit. If a team member already completed a wellness visit in a year, additional visits will not be required.


Tobacco Cessation

The Center for Tobacco Control (CTC) offers programs to help all employees live tobacco-free. The center has one of the highest success rates in the country, and offers individual and group programs located in Great Neck and Bay Shore. Participants in the program can receive group counseling, coaching and motivational text messages as needed. Employees and dependents also benefit from free cessation medications and tobacco alternatives to help achieve a tobacco-free life.

Northwell’s online well-being resource, myBottleCap is an innovative way that you and family members can assess, address and perhaps change your drinking habits so you can be comfortable with your well-being journey, now and in the future.

After a brief assessment gauging the frequency and amount of your alcohol intake, myBottleCap will provide you with an eight-week program that best fits your assessment’s results. Resources and support comes in the form of text messaging, or texting messaging combined with a personal coach. This program is 100% confidential. myBottleCap is a Northwell-owned resource through our Center for Addiction Services and Personalized Interventions Research (CASPIR). A research center designed to help people who experience problems as a result of their drinking, is developing and investigating new and innovative ways for people to reduce or stop drinking.
